You must be able to answer “YES” to every item below.
If not, do not submit an application.

☐ U.S. or Canadian resident
☐ At least 18 years old and legally able to enter into contracts
☐ Ready for a puppy in 2026 or 2027
☐ Secure, private fenced yard (not an apartment or shared/public yard, unfenced acreage is often okay)
☐ Able to pick up in person in Phoenix, AZ (driving, not flying if puppy is too large for in cabin)
☐ Able to afford the $5,000 purchase price by cashiers check with signed contract
☐ Accepts cropped & docked puppies and required ear posting aftercare
☐ Agrees to breeder-selected puppy placement (based on best fit, not first-come first-served)
☐ Agrees to lifetime return-to-breeder clause in the contract (may not rehome elsewhere, only return to breeder)
☐ Application → Possible interview → Possible waitlist acceptance
☐ Being on the waitlist does not guarantee a puppy
